Voluntary data use and consent to the processing and transfer of personal data for neurovantage research study purposes

1. Introduction

With new data privacy regulations in place around the globe, Neuro Vantage, Inc. (“NVI”) takes seriously the policies and procedures regarding your information and privacy and its obligation to be compliant with current privacy laws. This Data Use and Consent to the Processing and Transfer of Personal Data for Research Study form (the “Consent Form”), allows NVI to collect, process, store, and share certain pieces of your personal information as necessary to conduct academic and business related research study services on behalf of NVI clients in accordance with our policies and procedures regarding data privacy and data processing. At NVI, your privacy is of the utmost importance to us. Although we do collect certain limited personal information from study participants for legitimate business purposes, the information we collect is anonymized and/or de-identified to fully protect any potential sensitive personally identifiable information.

2. Consent to data processing

I, the undersigned, as the data subject, voluntarily give my explicit consent to NVI as the data controller, to the processing and transfer of my personal data in accordance with applicable data privacy laws and regulations under the following conditions:

3. The purpose of data processing

NVI will process my data outlined in Section 2, Subsection B of this Consent Form for the purposes relating to the associated biometric, behavioral and/or survey study for academic and/or commercial marketing research purposes. The insights gained from any study may be shared in perpetuity with NVI clients or other trusted third parties to help improve products, services, brand strategies, or customer experiences. NVI may receive compensation in exchange for conducting research and providing these insights.

4. The scope of data processed by the data controller

For the purposes mentioned in Section 2, Subsection A above, NVI will process the following data based on my consent:

• Full name;
• Contact details (phone number, email address, home address);
• Biometric data;
• Behavioral data;
• Survey-related data; and
• Any other personal data provided by me, as the data subject, prior to and/or during the research study.

The legal basis for processing

The legal basis for the processing of personal data is voluntary consent. NVI will process the data taking into full account the interests and fundamental rights of the data subject.

5. Declaration

I declare that I have read and understood NVI’s policies and procedures regarding data privacy as made available here.

I declare that I have been properly informed by NVI about the processing of my personal data and the de-identification process. I confirm that I fully understand my rights and have had an opportunity to ask any questions about NVI.

In light of the above, I give my explicit consent to the processing of my personal data by NVI.

This consent is given of my own free will, with full understanding of the purpose of data collection and its analysis, storage and sharing. As the data is de-identified as it is processed the data subject understands once the data is processed NVI will not be able to extract any personally identifiable information and therefore will be unable to confirm destruction or deletion.

In accordance with applicable data privacy law, I am aware that I can decide at any time to withdraw my consent for any future data collection by sending an email to info@theneurovantage.com, or by sending a letter to the address of NVI.
